Model Withdrawal Form
This document is a draft pending review by qualified counsel and pending registration of the operating entity. It is published in this state deliberately — an unreviewed legal page marked as such is more useful than a confident-looking one that has never been checked.
This is the model form set out in Annex I(B) of Directive 2011/83/EU. Using it is optional: any unambiguous statement of your decision to withdraw is enough. See the right of withdrawal notice for the period and its effects.
Where to send it
The addressee block cannot be completed yet. The operating entity is not registered, so there is no trading name, no geographic address and no email address to print here. Both are published in the imprint before the first order can be placed, and this form is completed with them at the same time.

The statutory right of withdrawal applies to consumers. Where you order in the course of a trade, business, craft or profession, it does not apply and this form has no effect. The terms of sale set out what does.
